Our vision is for all young people (14 to 25 years) who require additional support get the information and help they need to live the lives they want.


  • Facilitate partnerships and collaboration that puts young people at the centre of their support
  • Lead the implementation of the ‘Principles of Good Transitions’
  • Empower young people to be the voices of change

The Scottish Transitions Forum began in 2008 and currently there are over 850 members representing a wide range of professionals, young people, parents and carers. Membership is free and is open to anyone who is committed to improving transitions for young people with additional support needs.
